Constipation in the elderly

Description

 Запор у пожилых. Это нарушение стула у пациентов 60 лет и старше, при котором частота дефекаций составляет менее 3-х раз в неделю или опорожнение кишечника требует дополнительных усилий. Симптом чаще обусловлен естественными возрастными изменениями кишечной стенки, но может вызываться и органическими заболеваниями пищеварительного тракта. Для выяснения причин запора проводят ультразвуковое, рентгенологическое и эндоскопическое обследование, лабораторные анализы. Для нормализации состояния пожилым людям назначают растительные отвары, осмотические слабительные, ректальные суппозитории.

 Согласно статистическим данным, около 60% людей после 60 лет страдают из-за нарушений дефекации. Запоры значительно ухудшают качество жизни и способствуют возникновению органических патологий нижних отделов ЖКТ. У подавляющего большинства пожилых задержка стула обусловлена естественными изменениями нервной регуляции и возрастной дистрофией гладкой мускулатуры пищеварительного тракта. Основные физиологические причины нарушения нормальной частоты дефекаций:
 • Атония кишечника. Для людей пожилого возраста с ослабленным тонусом кишечной стенки характерно отсутствие кала в течение 3 дней и более, не сопровождающееся другими диспепсическими симптомами. Из-за расстройств иннервации пациенты не ощущают позывов к испражнению. Чтобы опорожнить кишечник, требуются усилия. Часто больные прибегают к различным манипуляциям - надавливают руками на промежность, разводят ягодицы в стороны, что способствует выделению кала.
 • Гиподинамия. Запоры возникают у пожилых людей, которым по медицинским показаниям необходимо придерживаться постельного режима. Как правило, самостоятельное отхождение кала затруднено или невозможно. На фоне длительной задержки каловых масс отмечается вздутие живота или боли в левой подвздошной области. Дефекация чаще наступает только после очистительной клизмы, масляных микроклизм или слабительных препаратов.
 • Слабость мышц пресса. Нормальное опорожнение нарушается из-за дистрофических и дегенеративных изменений мускулатуры брюшной стенки. Позывы ощущаются, но выделение кала затруднительно. В тяжелых случаях больные вынуждены прибегать к мануальной помощи. При дефекации приходится натуживаться, чрезмерное напряжение может сопровождаться резкой болезненностью в области заднего прохода.
 • Особенности диеты. Пожилые люди испытывают проблемы с опорожнением после приема большого количества сдобной выпечки, молочных каш, белого хлеба. Обычно кал отсутствует на протяжении 2-3 дней, после чего дефекация происходит самостоятельно с выделением сухих испражнений в виде комочков. Задержка стула также бывает обусловлена употреблением нескольких чашек крепкого чая в день, перееданием.
 • Расстройства эндокринной системы. У пожилых выделяют особую категорию нарушений - эндокринный запор, связанный с возрастной гипофункцией щитовидной, паращитовидной и половых желез. Самостоятельное опорожнение наблюдается крайне редко (1-2 раза в неделю), больные жалуются на тяжесть и дискомфорт слева в подвздошной области. При дефекации выделяются твердые комки кала, которые травмируют слизистую оболочку прямой кишки.
 • Стрессовые ситуации. Из-за нарушения скоординированной работы разных структур мозга и вегетативной нервной системы запоры у пожилых становятся реакцией на сильные эмоциональные потрясения. Многие пациенты жалуются на задержку кала после изменения привычных условий жизни: переезда, госпитализации в стационар. Эти расстройства кратковременны и хорошо поддаются лечению немедикаментозными методами.

Проктологические заболевания.

 Различные патологии прямой кишки сопровождаются запорами, которые вызваны не только механическими препятствиями к продвижению каловых масс, но и рефлекторным спазмом сфинктера в ответ на болевые ощущения. Из-за страха боли при опорожнении многие пожилые люди сдерживают позывы, что еще больше усугубляет ситуацию. При проктологических болезнях задержка стула может сочетаться с выделением слизи и крови из прямой кишки как при акте дефекации, так и в течение дня. К возникновению запора чаще приводят:
 • Геморрой. Пациенты испытывают трудности с испражнением, возможно появление субъективного ощущения неполного опорожнения кишечника. Иногда при сильном натуживании больные замечают выпадение красно-синюшных образований из анального отверстия (варикозных узлов). Со временем запоры становятся хроническими, стул отсутствует в течение 7 дней и дольше. Дефекация сопровождается интенсивными болями.
 • Проктит. Нарушение дефекации характеризуется кратковременными запорами, после чего всегда происходит самостоятельное выделение кала, требующее повышенных усилий. При выделении стула пожилые люди отмечают усиление болевого синдрома. В зависимости от степени воспаления задержка кала может чередоваться с частыми ложными позывами на дефекацию, при которых отделяются слизь и кровь.
 • Полипы прямой кишки. Запоры при опухолевых образованиях связаны с частичной кишечной непроходимостью. Типично постепенное урежение частоты опорожнения кишечника. Кал выделяется небольшими комками, при выраженном сужении просвета кишки формируется «карандашный» или лентовидный кал. Аналогичный вариант запора возникает у пожилых в случае приобретенных дивертикулов прямой кишки.
 • Спаечная болезнь. У пожилых больных запоры могут вызываться передавливанием участка кишки соединительнотканными перемычками. Длительность задержки стула колеблется от нескольких дней до недели и больше. В случае самостоятельного опорожнения выделяется в небольшом количестве сухой кал. Испражнение сопровождается резями в заднем проходе из-за травматизации слизистой.

Diagnostics

 Выяснением причин запора у пожилых занимается врач-гастроэнтеролог. Диагностический поиск предполагает комплексное обследование органов ЖКТ, которое направлено на изучение морфологических особенностей и функциональных возможностей пищеварительной системы. При необходимости назначают консультации других специалистов. Наиболее информативными считаются:
 • Ректальный осмотр. Пальцевое исследование прямой кишки производится для изучения структуры слизистой оболочки, определения увеличенных геморроидальных узлов, трещин и объемных образований. По показаниям обследование дополняют проведением аноскопии или ректороманоскопии для визуализации нижних отделов толстого кишечника.
 • Эндоскопия. Колоноскопия назначается всем пожилым людям с жалобами на задержку дефекации для детального изучения структуры ободочной и сигмовидной кишки, выявления опухолевых новообразований. При обнаружении подозрительных участков осуществляется биопсия для дальнейшего гистологического анализа.
 • Рентгенологическое исследование. Выполнение рентгеновских снимков после перорального контрастирования ЖКТ сульфатом бария необходимо для обнаружения органических изменений кишечной стенки, признаков язвенных дефектов, воспалительных процессов. Отсроченные снимки делают для определения скорости пассажа химуса по кишечнику.
 • Сонография. При помощи УЗИ брюшной полости оцениваются структурно-функциональные особенности пищеварительного тракта, выявляются патологические изменения, способные спровоцировать запор. При необходимости производится УЗИ крупных сосудов брюшной полости для исключения окклюзии мезентериальных артерий.
 • Анализы кала. Запоры у пожилых сопровождаются изменением консистенции и состава каловых масс, что устанавливается с помощью копрограммы. Для изучения микрофлоры толстого кишечника осуществляют бактериологический анализ. Исключить хронические кровотечения из ЖКТ помогает реакция Грегерсена на скрытую кровь.
 Для уточнения диагноза и детальной визуализации структур брюшной полости назначают компьютерную томографию. Всем пациентам проводят общий и биохимический анализы крови. При подозрении на поражение кишечной сосудистой сети выполняют ангиографию с контрастированием. Для исключения заболеваний гепатобилиарной системы используют дуоденальное зондирование с микроскопическим и бактериологическим исследованием желчи.

Treatment

 Если запор связан с возрастными изменениями организма, для нормализации стула в первую очередь применяют немедикаментозные методы. Рекомендуется включить в рацион богатые клетчаткой продукты: цельнозерновой хлеб, свежие или тушеные овощи. Для восстановления естественной дефекации полезно выпивать около 2 литров жидкости в день. Чай лучше заменить кисломолочными напитками, компотами. Для стимуляции перистальтики пожилым людям нужно по утрам съедать кусочек хлеба со сливочным маслом или выпивать 1 ложку растительного масла.
 Для укрепления мышц брюшного пресса и ускорения продвижения каловых масс рекомендовано выполнять специальный комплекс физических упражнений. Чтобы устранить запоры, можно использовать растительные препараты - отвары коры крушины, травы тысячелистника. С целью быстрого очищения кишечника назначают клизмы. При упорных запорах применяют пероральные слабительные средства, ректальные суппозитории. При задержке стула более 3-х дней, которая сопровождается интенсивной болью в полости живота, нарушением общего состояния, необходимо как можно быстрее обратиться к врачу.
