Subfebrile temperature

Description

 Субфебрильная температура. Это повышение температуры в пределах 37-38 градусов Цельсия. Симптому сопутствуют озноб, мышечные и головные боли, разбитость. Субфебрилитет встречается при вирусных и хронических бактериальных инфекциях, эндокринной патологии, злокачественных новообразованиях, других заболеваниях. Чтобы выяснить, почему повысилась температура, используют бактериологические и серологические исследования, УЗИ, высокоинформативные визуализирующие методы. Температурные показатели нормализуются при правильно подобранном лечении основной болезни.

Normal values

 Подъем температуры до субфебрильной днем наблюдается при интенсивных спортивных тренировках, тяжелой физической работе. В подобных случаях ключевые причины температурных колебаний - усиленные сокращения мышечных волокон и повышенная выработка тепла. Обычно показатели приходят в норму через 1-2 часа. Если этого не произошло, и общее состояние ухудшилось, стоит обратиться к врачу. Субфебрилитет возможен после длительного пребывания на солнце, особенно у маленьких детей, терморегуляторные механизмы которых несовершенны.

Reasons

 • Поствирусная астения (температурный «хвост» различных инфекций).
 • Очаги хронической инфекции. Воспаление придатков, кариозные зубы, инкапсулированные абсцессы.
 • Анемии. Железодефицитная, гемолитическая, апластическая.
 • Патологии мочевыделительной системы. Уретрит, цистит, хронический пиелонефрит.
 • Вегето-сосудистая дистония, термоневроз.
 • Хронический сепсис.

Эмоциональные реакции.

 Любые изменения в коре головного мозга вследствие психоэмоциональных реакций приводят к кратковременным нарушениям работы гипоталамического центра терморегуляции. Такое эмоциональное состояние сопровождается субфебрильной температурой тела, что более характерно для женщин, склонных к истерическим реакциям. Субфебрилитет может сохраняться долгое время при хронических стрессах, неврозах и неврозоподобных состояниях. Человек плохо переносит повышение температуры, его беспокоят постоянная слабость и разбитость, апатия, снижается аппетит.
 • Поражение уха. Евстахиит, лабиринтит, наружный и средний отит.
 • Воспаление горла. Катаральная ангина, фарингит или ларингит, острый и хронический тонзиллит.
 • Болезни носа и околоносовых пазух. Обострение хронического ринита, вазомоторный ринит, синусит.
 • TORCH -инфекции. Цитомегаловирус, простой герпес, вирусы Коксаки.

Diagnostics

 Субфебрильная температура регистрируется при множестве заболеваний, поэтому первичной диагностикой зачастую занимается врач-терапевт. Его задача заключается в оценке всех систем организма, выявлении нарушений, которые могли бы привести к температурному подъему. Обследование включает расширенные лабораторные анализы крови и других биологических материалов, современные инструментальные методы. Наибольшей диагностической ценностью обладают:
 • Анализы крови. При оценке общего анализа обращают внимание на уровень лейкоцитов и соотношение разных лейкоцитарных фракций, количество гемоглобина и эритроцитов. В биохимическом исследовании крови обнаруживают признаки воспаления (С-реактивный белок, сиаловые кислоты). Печеночные пробы позволяют определить, может ли субфебрильная температура быть связанной с вирусным или другим поражением печени.
 • Бактериологические методы. Для выяснения причины длительной субфебрильной температуры выполняется трехкратный забор крови и ее посев на селективные питательные среды. При симптомах респираторной инфекции исследуется мазок из зева, образцы мокроты и слизи из носа. Для идентификации туберкулеза необходимо собирать утреннюю и вечернюю мокроту для посева на среду Циля-Нильсена.
 • Серологические реакции. Для уточнения связи субфебрильной температуры с вирусными инфекциями пользуются точными методами ИФА, РИФ, ПЦР. Эти анализы широко применяются для обнаружения маркеров вирусных гепатитов, антител к возбудителям кишечных и респираторных заболеваний. Для исключения ВИЧ проводится иммуноблоттинг, качественные ПЦР.
 • Сонография. Ультразвуковое исследование служит скрининговым способом оценки состояния органов брюшной полости, эндокринных желез, суставов. Женщинам рекомендовано УЗИ молочных желез, опухоли которых часто являются причиной субфебрильной температуры. Обязательно выполняют УЗИ печени для исключения деструктивных и воспалительных процессов.
 • Рентгенологическая визуализация. Субфебрильная температура возникает при патологии кишечника, поэтому информативна ирригоскопия с бариевой взвесью или полное рентгенологическое исследование пищеварительного тракта с контрастированием. Для подтверждения синуситов выполняют рентгенограммы придаточных пазух носа. Всем больным с субфебрилитетом назначают рентгенографию легких.
 • Инвазивные исследования. При дерматомиозите и склеродермии иногда требуется биопсия пораженного участка мягких тканей для гистологического исследования. Если субфебрильная температура сочетается с симптомами интоксикации и лимфаденопатией, проводят биопсию лимфоузла с последующим цитологическим исследованием. Биопсия кишечника показана для дифференциальной диагностики ВЗК.
 • Уточняющие методы. У женщин исследуют гормональный профиль, чтобы исключить патологический климакс, беременность. Субфебрильная температура при заболеваниях дыхательных путей требует проведения спирографии для оценки функции внешнего дыхания. Пациентам с экссудативными диатезами исследуют расширенную иммунограмму и аллергологические пробы.

Treatment

 Среди врачей общепринято мнение, что при субфебрильной температуре необходимо воздействовать на причины состояния, а использование жаропонижающих препаратов считается нецелесообразным. Антипиретики назначают в исключительных случаях маленьким детям, которые тяжело переносят лихорадку и подвергаются риску развития судорожного синдрома. После выяснения причины длительного субфебрилитета назначаются следующие группы препаратов:
 • Антибиотики. Когда субфебрильная температура вызвана вялотекущими инфекциями, необходимы длительные курсы приема антибактериальных средств, подобранных с учетом вида патогенной микрофлоры. При туберкулезе комбинируют 3-4 специфических препарата в течение 8-12 месяцев.
 • Противовирусные лекарства. Этиотропные препараты подходят для лечения вирусных гепатитов, сопровождающихся субфебрильной температурой, ВИЧ-инфекции. Лекарственные средства угнетают размножение возбудителя и снижают вирусную нагрузку в крови. При респираторных и кишечных инфекциях не используются.
 • Нестероидные противовоспалительные средства. При ревматических болезнях эффективны медикаменты, которые уменьшают синтез цитокинов, снимают боль и скованность движений. В случае продолжительной терапии выбирают селективные ингибиторы ЦОГ-2 для снижения побочных эффектов со стороны ЖКТ.
 • Витамины. Для укрепления организма и ускорения выздоровления назначаются аскорбиновая кислота, витамины группы В. Полезны природные антиоксиданты - токоферол, ретинол. При общем истощении принимают метаболические препараты, которые обеспечивают организм энергией.
 • Гормональные средства. Эстрогенные препараты рекомендованы женщинам в климактерическом возрасте. Они устраняют негативные проявления приливов, субфебрильная температура после курса гормонов исчезает. Комбинированные контрацептивы оказывают хорошее терапевтическое действие при тяжелом предменструальном синдроме.
 • Цитостатики. Антиметаболиты показаны в составе комбинированной химиотерапии онкопатологии. В большинстве случаев их применяют одновременно с лучевой терапией. Цитостатики принимают при тяжелых аутоиммунных процессах, рефрактерных к лечению кортикостероидами.
 • Инфузионные растворы. При тяжелом течении бактериальных и вирусных инфекций с целью дезинтоксикации могут потребоваться внутривенные вливания кристаллоидных и коллоидных растворов. Для предотвращения отека головного мозга средства комбинируют с петлевыми диуретиками.
 Субфебрильная температура в сочетании с кровянистыми испражнениями при обострении воспалительных заболеваний кишечника требует иссечения язв или резекции кишки с формированием анастомоза. Опухоли из лимфоидной ткани являются показанием к оперативному вмешательству - радикальной лимфаденэктомии после массивной химиотерапии. При лейкозах успешно практикуют пересадку костного мозга, которая позволяет восстановить нормальный клеточный состав крови и предотвратить тяжелые инфекционные осложнения.

 Сохраняющаяся в течение нескольких дней субфебрильная температура тела - это состояние, требующее незамедлительного обращения к специалисту для верификации причины. Запрещено принимать жаропонижающие средства, поскольку они мешают организму бороться с инфекцией и смазывают клиническую картину. При симптомах интоксикации необходимо обильное теплое питье (вода, соки, травяные чаи), пища должна быть легкоусвояемой и витаминизированной. Нужно ограничить физическую активность, при общем недомогании желательно соблюдать постельный режим.
